• This information collection form is designed to optimize the account opening experience; it is not designed for the opening of accounts with minor children or non-US Citizens as owners, business accounts, fiduciary accounts (eg Representative Payee accounts) or any account requiring the verification of legal documents (eg Power of Attorney, Trust Accounts, Estate Accounts...).
